On or about December 7, 2016, roof area - Building 120, employees installing roof shingles on a 3:12 pitch roof were not protected from falling 32 feet by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about December 7, 2016, the Werner 40 feet extension ladder being used was extended 3 feet past the landing but was not secured to prevent accidental displacement. Employees were being exposed to a 32 feet fall hazard.
