29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about December 12, 2016, Ernesto Teran-Quiñonez did not ensure that each employee(s) engaged in steep roofing activities 6 feet or more above lower levels were protected by guardrails, safety nets, or personal fall arrest systems. Employees of Ernesto Teran-Quinonez were exposed to fall hazards of up to 11 feet while engaged in roofing activities on a one- story commercial building with no fall protection in place.    Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Ernesto Teran-Quiñonez, aka, Ernesto Teren was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 981835, citation number 1, item number 1 and was affirmed as a final order on September 18, 2014, with respect to a workplace located at 2505 N Weber Colorado Springs, CO 80907.
