29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.    On or about 15 December 2016, the employer failed to fully protect the employees from a fall hazard when the employees were required to climb onto the Bowling Pin machine that was 4 feet or more above the floor. This exposed the employees to a fall hazard.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(1): The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about 15 December 2016, the employer did not establish an energy control (lockout/tagout) program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance, the machine would be isolated and rendered inoperable. Employees exposed to caught-in hazards likely to result in crushing injuries and/or amputation from unexpected energizing, start up or release of stored energy from the bowling pin machines.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    On or about 15 December 2016, the employer failed to ensure that Pulley(s) with part(s) seven feet or less from the floor or work platform were guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o) on the Bowling Pin Machines. Employees were exposed to the hazard of being caught in an ingoing nip point.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    On or about 15 December 2016, the belt and pulley assemblies on the Bowling Pin Machines were unguarded.  Employees were exposed to the hazard of being caught in a ingoing nip point.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
