29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) On or about 12/12/16 at 1210 East Avenue, Rochester, New York; an employee was picking up debris on the ground without wearing a hard hat while three employees worked above him at a height of 18 feet tearing off a roof and edging materials and dropping them onto the ground below.  Employee was exposed to being struck in the head by pieces of wood with nails and roofing materials dropping from a height of 18 feet.         ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a) On or about 12/12/16 at 1210 East Avenue, Rochester, New York; employees, involved in tearing off the roof and edging on the south side of a residential site using a ladder jack scaffold, were exposed to a fall of 18 feet when the ladder they were using to access the scaffold extended approximately 18 inches above the landing surface.      NO ABATEMENT CERTIFICATION REQUIRED
