29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which were used by employees did not have approved ground-fault circuit interrupters for personnel protection (NOTE: Receptacles on a two-wire, single-phase portable or vehicle-mounted generator not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.):  a) On January 3, 2017, employees were provided with electrical tools and equipment connected by cord and plug without the use of an assured equipment grounding program or Ground Fault Circuit Interrupters (GFCI's).

29 CFR 1926.416(a)(1): The employer permitted employees to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about December 23, 2016 and January 4, 2017, employees performing carpentry work during the new construction of a single-family home were permitted to work in proximity to energized overhead primary power lines of 12,000 volts, thereby exposing employees to electric shock and fall hazards of approximately twenty-four (24) feet.
