29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    Located at 391 Varnum Ave. Lowell, MA:      On or about 12/19/16, the employer did not record the following workplace injury and illness on the OSHA 300 log for calendar year 2016;    a) On or about 12/19/16, a case manager leaving the adult-1 unit was punched and scratched in the face, eye, back, and body by an aggressive patient, causing a black eye, scratched cornea, and bruising all over the body. The injury resulted in days away from work.    UHS of Westwood Pembroke, Inc. dba Lowell Treatment Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.4(a), which was contained in OSHA inspection number 1009746, citation number 2, item number 1a and was affirmed as a final order on 5/27/2016, with respect to a workplace located at 391 Varnum Ave., Lowell, MA.
