29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about January 10, 2017, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance activities on the H2 Roll Coater Line, UZ Coating Line, and other machines and equipment at the facility were protected from unexpected energizing, startup, or release of stored energy. Machines and equipment used throughout the lines are powered by electric, pneumatic, thermal, natural gas, and hydraulic power.

29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. On or about January 10, 2017, the employer did not conduct frequent (daily to monthly) inspections of cranes, including manually and electrically powered wall and overhead cranes. Cranes are used on a daily basis in the compounding area, H2 Coating Line entry and exit, UZ Coating Line entry and exit, inspection area, and other areas throughout the facility.

29 CFR 1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: On or about January 10, 2017, the employer did not conduct periodic (annual) inspections of cranes, including wall and overhead cranes, since on or about August, 2015. Cranes are used on an almost daily basis in the compounding area, H2 Coating Line entry and exit, UZ Coating Line entry and exit, inspection area, and other areas throughout the facility.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about January 10, 2017, the employer did not ensure one or more methods of machine guarding were provided to protect the operator and machine helper employees from the hazards created by the ingoing nip points and rotating parts of the material traveling over and through coater rollers of the H2 Coating Line. b. On or about January 10, 2017, the employer did not ensure one or more methods of machine guarding were provided to protect the operator and machine helper employees working from the hazards created by the ingoing nip points and rotating parts of the material traveling over and around the wind-up rollers of the UZ Coating Line.
