29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:     a)  Front of House - On or about January 12, 2017, masons were parging a brick wall from the work platforms of a fabricated frame scaffold that was only 12 inches wide and did not completely span the area between the scaffolds vertical uprights thereby subjecting a mason to a 13 foot fall.    b)  Front of House - On or about January 12, 2017, masons were parging a brick wall from the work platforms of a fabricated frame scaffold that was only 12 inches wide and did not completely span the area between the scaffolds vertical uprights thereby subjecting a mason to a 4 foot fall.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:   a)  Front of House - On or about January 12, 2017, employee was observed climbing down scaffold using cross bracing as footing .   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a)  Front of House - On or about January 12, 2017, a mason was observed not provided with fall protection and  exposed to a 13 fall from an unguarded platform of a fabricated frame scaffold. 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     a)  Front of House - On or about January 12, 2017, workers  without head protection were observed exposed to a struck-by hazards from materials, including, but not limited to, buckets containing lime paste, that were not restrained or otherwise prevented from falling  from a scaffold platform on to the employees working below. 29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.   a)  Front of House - On or about January 12, 2017, materials, including, but not limited to a bucket containing lime paste, was not restrained or otherwise prevented from falling  from a scaffold platform down upon the employee working on platform below.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
