29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows: a) Front of House - On or about January 12, 2017, masons were parging a brick wall from the work platforms of a fabricated frame scaffold that was only 12 inches wide and did not completely span the area between the scaffolds vertical uprights thereby subjecting a mason to a 13 foot fall. b) Front of House - On or about January 12, 2017, masons were parging a brick wall from the work platforms of a fabricated frame scaffold that was only 12 inches wide and did not completely span the area between the scaffolds vertical uprights thereby subjecting a mason to a 4 foot fall. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access: a) Front of House - On or about January 12, 2017, employee was observed climbing down scaffold using cross bracing as footing . In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
