29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  (a) Second floor, east side of the re-saw building: Employees walking to work locations and/or installing structural steel components proximal to a partially decked area  were  not protected from falls of 15' 4" onto concrete.   (b) Second floor, south side of the re-saw building: Employees walking in the area  and/or  prepping structural steel components near the opening in the wall used to pass materials were not protected from falls of 15' 4" onto concrete and construction materials   Note: Abatement certification and documentation ARE required for this item.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in 1926.502  (a)  Roof of re-saw building: On or before January 20, 2017 up to seven workers removing snow from the roof were connected to a horizontal life line  which had not been designed, installed, and used, under the supervision of a qualified person.  (b)  Roof of re-saw building:  On or before January 20, 2017 the Miller "Mighty Lite" self-retracting lifeline was not taken out of service after the impact indicator had been activated.
