(a) Wyvern Site, Track Location OOC 418.1 - On or about January 24, 2017, the employer exposed employees to an engulfment hazard, in that employees working in an unprotected trench, ranging from 6 to 9 feet deep with walls that had been cut almost vertically, were not provided with retaining devices to prevent soil from falling into the excavation. The spoil pile was placed at the edge of the north wall.

The near vertical excavated walls were not sloped, shored, or shielded.
