On or about January 23, 2017 and after where the employer did not record a work related illness which resulted in lost time away from work as order by Alberto Flores , MD with a return to work day of January 26, 2017 and no OSHA 300 form or equivalent for the calendar year 2017 was produced from the employer during the inspection walk around.
