29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      At the jobsite, employees operating Hitachi nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.      Lorenzo Quintero, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 975247 Citation Number 1, Item number 3, and was affirmed as a final order on June 3, 2014, with respect to a workplace located at 2816 England Parkway, Grand Prairie, Texas 75126.

At the jobsite, employees engaged in framing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 20 feet to the ground below.      Lorenzo Quintero, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 975247, Citation Number 1, Item number 5, and was affirmed as a final order on June 3, 2014, with respect to a workplace located at 2816 England Parkway, Grand Prairie, Texas 75054.
