29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On May 19, 2024, and times prior thereto, employees work from a pole scaffold without a fall protection system while installing cornice material on the exterior of a residential home. The employees were exposed to fall hazards of approximately 14 feet to the ground below.

29 CFR  1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.   On May 19, 2024, and times prior thereto, employees working from pole scaffolds without cross bracing installed were exposed to a collapse and falling approximately 14 feet to the ground below.

On May 19, 2024, and times prior thereto, employees engaged in cornice work on a residential home from single pole scaffolds, work from platforms that are not 18 inches wide. The employees were exposed to potential fall hazard of approximately 14 feet to the ground below.
