The employer had not maintained a powered industrial truck program that provided training, certification, and re-certification since 1997. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative:   a) Facility: On February 9, 2017 and at times prior thereto the employer had not developed, implemented or maintained an energy control program that was relative to the current machinery in use.  Specific, written procedures that identified all of the hazardous energy sources, how and where to control them were not developed.  Training for authorized and affected employees was not current.  Periodic inspections were not conducted and certified respective to the current machinery.  Employees performed, or assisted in the performance of scheduled and unscheduled servicing and/or maintenance on machinery that included but may not be limited to the fan fold & cutoff system and the Nuova idro-press block moulding machine.
