29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 1408 Termon Avenue, Pittsburgh, PA: On or about February 13, 2017, during roofing activities, the employer did not ensure that the ladder used to gain access to/egress from main roof of the home to a lower balcony area was properly extended or that it was secured and that a grab rail was installed. The ladder in use was extended approximately 1 1/2 feet above the roof line and there was nothing in place to prevent it from moving nor was a grab rail installed.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Job site, 1408 Termon Avenue, Pittsburgh, PA: On or about February 13, 2017, during roofing activities, the employer did not ensure that the employees wore appropriate head protection where was a possible danger of being struck by objects from above.
