29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) East side of building - On or about February 17, 2017 the employer failed to have a lockout procedure and training for the CRS cut off machine that resulted in an amputation during a routine maintenance procedure.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:    (a) Lumberton, MS. - On or about February 17, 2017 an employee had the tip of two fingers amputated and employer failed to report the incident within the required 24 hours.
