29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about February 15, 2017 and at times prior, the employer, Excalibur Machine & Sheetmetal, Inc., did not establish and implement an energy control program so as to protect employees, who were required to clean and maintain metal fabrication and milling machines, from the unexpected energizing, startup or release of stored energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 15, 2017 and at times prior, the employer, Excalibur Machine & Sheetmetal, Inc., did not ensure employees, who were required to operate a Dayton belt sander, were protected from in-running nip points. b) On or about February 15, 2017 and at times prior, the employer, Excalibur Machine & Sheetmetal, Inc., did not ensure employees, who were required to operate a STARTRITE H225 horizontal band saw, were protected from amputation hazards. c) On or about February 15, 2017 and at times prior, the employer, Excalibur Machine & Sheetmetal, Inc., did not ensure employees, who were required to operate an Amada RG80 press brake, were protected from crushing hazards. d) On or about February 15, 2017 and at times prior, the employer, Excalibur Machine & Sheetmetal, Inc., did not ensure employees, who were required to operate an Amada 8025 press brake, were protected from crushing hazards.
