29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     (a)  2064 South Queen Street, York, PA  - Employees were observed performing roofing activity on a low-slope roof 4 in 12 pitch, without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 22 feet to the ground below, on or about March 2, 2017.      No abatement certification or documentation required.    ----------------------------------------------------------------------------------------------------------------------------------  Per ISA 4/12/17    modify AVD from employees to subcontractor employees.

29 CFR 1926.1053(b)(4):  Ladders were used for purposes other than the purpose for which they were designed:    (a)  2064 South Queen Street, York, PA -  The employees were using a 8 foot Werner fiberglass step ladder in the closed position leaning up against the side wall being used for access approximately 6 feet high to a light pendent, thereby exposing the employee to injuries from ladder collapse or deflection, on or about March 2, 2017.
