The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard.  (a) M/V Bell M. Shimada - fantail: On or about February 28th, 2017 and at times prior thereto the CSHO observed an employee grinding a padeye on the port side without a guard on the handheld right angle grinder.

Shimada- aft of engineering: On or about February 28th, 2017 and at times prior thereto, the CSHO observed a DeWalt Bench Grinder (DW758) with an upper adjustable tongue guard measuring 1" from the peripheral edge of the grinding wheel.  (b) M/V Bell M. Shimada- port side engineering: On or about February 28th, 2017 and at times prior thereto, the CSHO observed a JET Bench Grinder(JBG-10A) with an upper adjustable tongue guard measuring 1 3/8" from the peripheral edge of the grinding wheel.

Shimada- aft of engineering: On or about February 28th, 2017 and at times prior thereto, the CSHO observed a DeWalt Bench Grinder (DW758) with grinding wheel that was loaded with foreign matter. The hazardous condition may cause the wheel to explode severly injuring employees.
