29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) on or about 2/28/2017 and at times prior thereto, employees were exposed to caught in hazards in that the Colchester lathe was not guarded.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  (a) on or about 2/28/2017 and at times prior thereto, an oxygen cylinder on the weather deck of the Shimada was not separated from a fuel-gas cylinder.

29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  (a) on or about 2/28/2017 employees were exposed to crushing hazards in that an employee was operating a powered industrial truck in a warehouse and on the docks and was not wearing a seat belt.
