The panel door was missing the latch to keep it closed which exposed employees to electrical hazards.  b) At the facility, where the relocatable power tap was not used in accordance with the listing and labeling.  The relocatable power tap was affixed to the pegboard near the Atec Non Destructive Testing (NDT) Machine, model number 15NDTC, serial numbers 2161012.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) At the facility on March 8, 2017, where the orange flexible cord that supplied electrical power to the portable industrial fan was not inspected for defects prior to use.  The flexible cord insulation was spliced and the conductors were held together with a piece of tape.
