29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degree F (37.8 degree C), were dispensed into containers without the containers being electrically interconnected: On or about April 3, 2017, employees transferring n-butyl alcohol, which is a Category 3 flammable liquid with a flashpoint of 84 °F, were exposed to potential fire and explosion hazards in that the nozzle and secondary container were not electrically interconnected. 29 CFR 1910.134(k)(5): Respiratory protection retraining was not conducted annually: On or about March 9, 2017, employees required to use respirators when performing tasks throughout the facility including, but not limited to etching and coating had not received respiratory protection training on an annual basis. Training was last conducted on January 27, 2016.
