On or about 3/13/2017, an employee at Mid-America Precast, Inc., was operating a Caterpillar forklift, serial # 1CN90125, in need of repair, defective, and unsafe.  The forklift was missing a LPG locator/indexing pin, had a damaged LPG tank hold-down strap and an improperly installed LPG tank.  The forklift was not removed from service until restored to safe operating condition.

On or about 3/13/2017, Mid-America Precast, Inc. did not ensure that slings and all fastenings and attachments were inspected prior to utilization.  The following damaged or defective slings were not removed from service:    a)  No identification tag and proof load test on sling hanging on a Robbins & Myers 10 ton crane, Serial #46891VY2, located in the steel shop.    b)  An alloy steel chain hanging on the wall of the steel shop was missing hasps on all four hooks.    c)  The sling hanging on the P & H 5 ton crane, Serial #7299; in the west bay had kinked and bent cables and a nail inserted into a holding a pin.
