29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, was not worn and attached to the boom or basket when working from an aerial lift.                         This violation occurred on or about March 14, 2017,  the employer did not ensure that employees were wearing safety harnesses with lanyards attached to the proper anchor points while operating the Haulotte aerial lift exposing employees to a fall from approximately 30 ft.  to the ground below.
