29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.  a)  On or about March 16, 2017 at 5900 Co Rd NN in Denmark, the employer did not install guidelines for a path between the roof edge at the access point 18 ft. above surface level and the warning lines of the employees work area.

29 CFR 1926.503(a)(2)(viii):  The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the standards contained in this subpart.  a)  On or about March 16, 2017 at 5900 Co Rd NN in Denmark, the employer did not ensure employees were completely trained in the recognition of fall hazards when using monitor and warning line system as fall protection on a low slope roof 18 ft. above surface level.  Employees were using mechanical equipment, warning lines were not installed on all sides of the work area, and guidelines were not installed from the roof edge access point to the work area.
