On April 3, 2019 at 117 Main Street in De Pere, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazards associated with employees accessing/ egressing unprotected roof edges at heights greater than ten feet above surface levels without utilizing fall protection equipment.
