29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Kwiki Tire Service at 1680 S. Main, Delta, CO 81416: On and before March 24, 2017, the employer did not ensure each employee who was allowed to operate the Caterpillar Forklift, model V51B, onsite was provided with operator's safety training as required under the standard. Employees used the forklift to move tires. This condition exposed employees to struck-by hazards associated with forklift operations. Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

Main, Delta, CO 81416: On and before March 24, 2017, the employer did not ensure the powered industrial trucks were examined daily before being placed in service. Daily examinations were not conducted by employees that operated the Caterpillar forklift, model V51B. This condition exposed the employees to the hazard of operating forklifts without proper inspections.
