29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  On or about April 4, 2017, an employee was exposed to a fall hazard greater than 6 feet to the ground level below, while attaching roofing shingles, without using fall protection equipment.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 3, 2017, an employee was exposed to a fall hazard greater than 6 feet to the ground level below, while attaching roofing shingles without being trained to recognize the hazards of a fall.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  On or about April 3, 2017, an employee was exposed to a fall hazard of greater than 6 feet to the level below, while accessing the roof with a ladder whose side rails did not extend 3 feet above the landing area.
