29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  On or about  December 4, 2020, employees were exposed to a fall hazard greater than 6 feet to the ground below while attaching roof shingles without using a fall protection system.  Guadalupe Ramirez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1220733, citation number 1, item number 1, and was affirmed as a final order on May 15, 2017, with respect to a workplace located at 3106 Piedra De Rio San Antonio, TX 78259.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 4, 2020, employees were exposed to a fall hazard greater than 6 feet to the ground below, while attaching roofing shingles without being trained to recognize the hazards of a fall.  Guadalupe Ramirez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1) which was contained in OSHA inspection number 1220733, citation number 1, item number 2 and was affirmed as a final order on May 15, 2017, with respect to a workplace located at 3106 Piedra De Rio San Antonio, TX 78259.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  On or about December 4, 2020 employees were exposed to a fall hazard greater than 6 feet, while accessing the roof with a ladder whose side rails did not extend 3 feet above the landing.  The Guadalupe Ramirez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 1220733, citation number 1, item number 3 and was affirmed as a final order on May 15, 2017, with respect to a workplace located at 3106 Piedra De Rio San Antonio, TX 78259.
