A gap of approximately 1 and 1/2 inches existed between the abrasive wheel and the frame of the machine. Employees using the bench grinder where exposed to struck-by and fire hazards while using the bench grinder.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Welding area, 565 Cedar Way, Oakmont, PA 15139: Observed on or about March 28, 2017, the work rest on the right side of a Dayton 8 inch bench grinder (model number 2LKR9, serial number 13120690) was not adjusted such that the maximum opening between the abrasive wheel and the work rest was 1/8 of an inch or less. The gap between the abrasive wheel and the work rest was approximately 9/16 of an inch. Employees using the bench grinder were exposed to caught-by hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Welding area, 565 Cedar Way, Oakmont, PA 15139: Observed on or about March 28, 2017, an electrical outlet box (110 volt) being used to supply power an angle grinder and a Dayton 8 inch bench grinder was being used as a pendant style device. The outlet box lacked proper strain relief and was equipped with knockouts. The knockouts could be displaced by an employee's fingers during use. Employees using the outlet box were exposed to the hazard of electrical shock. b) Rebuild area, near red beam, 565 Cedar Way, Oakmont, PA 15139: Observed on or about March 28, 2017, an electrical outlet box (110 volt) handing near a red beam was being used as a pendant style device. The outlet box lacked proper strain relief to be used as a pendant style device. Knockouts on the sides of the outlet box were missing and taped over. Employees using the outlet box were exposed to the hazard of electrical shock.

No. 258-317) was not connected to the transformer so that tension would not be transmitted to joints or terminal screws. The flexible cord was being used to supply power to a Miller brand Millermatic 252 welding machine. Employees working in the area were exposed to the hazards of electric shock and fire. b) Shop area, on wall near Line 45, 565 Cedar Way, Oakmont, PA 15139: Observed on or about March 28, 2017, the conduit leading into a 110 volt outlet box labeled 'CIR 7' was not connected to the outlet box so that tension would not be transmitted to joints or terminal screws. Employees working in the area were exposed to the hazards of electric shock and fire.
