29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 dBA: a) In the Production Area, on or about December 12, 2019 - A continuing, effective hearing conservation program was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 dBA. An employee, designated as an Master Technician, had an exposure to continuous noise at a dose of 67.58%. The exposure is approximately 1.35 times the OSHA action level of 85 dBA. This is equivalent sound level of 87.1 dBA .

29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: a) In the Production Area, on or about December 12, 2019 - An employee was not refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation following a standard threshold shift. An employee, designated as an Master Technician, had an exposure to continuous noise at a dose of 67.58%. The exposure is approximately 1.35 times the OSHA action level of 85 dBA. This is equivalent sound level of 87.1 dBA .
