29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    Location: 2501 Pitkin Ave, second floor    On or about: 11 April 2017    a) Worker used the crossbraces of a tubular frame welded scaffold to climb from the second floor to the third floor..

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:      Location: 2501 Pitkin Ave, sidewalk shed.     On or about: 11 April 2017    a) Employees were standing close to the edge of a sidewalk shed that had one of the sides taken off to hoist materials.
