29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     a.  On or about April 13, 2017, at the back of the building and the roof, protective helmets were not worn by employees when working in the area below a crane's headache ball and sling which exposes employees to struck-by hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about April 13, 2017, at the upper roof of the building, the side rails of an extension ladder did not extend at least 3 feet above the upper landing surface and the ladder was not secured at its top to prevent deflection which exposes employees to fall hazards.
