29 CFR 1926.502(d)(16)(ii): Personal fall arrest systems, when arresting a fall, were not capable of limiting maximum arresting force to 1800 pounds (8 kN) when used with a body harness: a) On or about April 13, 2017, and at times prior, the employer did not ensure that North FP2/315 Self-Retractable Lifeline used as part of a personal fall arrestsystem were anchored vertically and positioned above the worker in accordance with the manufacturer's requirements. The self retractable lifeline was anchored on the floor level, below the worker and lying in a horizontal position on the work surface, which could result in a failure of the braking system and allow a free fall of 15 meters, the length of the lanyard. 29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about April 13, 2017, and at times prior, the employer did not ensure that employees using North FP2/315 Self Retractable Lifelines had anchored and positioned them correctly according to manufacturer's specifications. The horizontal positioning of the lifeline could cause the braking mechanism to fail, resulting in a free fall of 15 meters, the length of the steel cable lanyard.
