The employees are required to access the upper level mezzanine storage area to retrieved items pertaining to the shop area such as, but not limited to boxes and other excess shop materials.  The mezzanine area was not equipped with the standard guardrails along the open-sides of the storage area, exposing employees to a fall of 11' to lower level, on or about 04/19/17.

The company's written lockout/tagout specific procedures were found with the following deficiencies:          a.  The energy control procedures did not specifically outline the steps for placement of              locks onto the multi-energy source disconnect apparatus.          b. The energy control procedures did not specifically outline the steps for the removal of             locks and restoring the equipment back to service.           c.  Specific requirements for testing the machine or equipment to determine and verify              the effectiveness of lockout devices, tagout devices and other energy control             measures were not included in the specific procedures.
