29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition.     House of Cheatham, Inc., Stone Mountain, GA - In the production area - Oil/water/chemicals  were accumulating on the floor of the compound area and line 11 of the bundle area creating a slipping/tripping condition for employees walking and working around the equipment, on or about April 19, 2017.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   House of Cheatham, Inc., Stone Mountain, GA  - In the production area, employees were required to use half mask/full face, tight fitting respirators for protection against chemicals.  A written respiratory protection program had not been updated, on or about 04/19/17.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:     House of Cheatham, Inc., Stone Mountain, GA  - In the production area, employees were required to use half mask/full face, tight fitting respirators for protection against chemicals.   Respirators were observed stored improperly and not cleaned.  A written recommendation regarding the employee's ability to use the respirator from the PLHCP was not conducted, on or about 04/19/17.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
