29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:    a.  On or about April 20, 2017, at the above addressed jobsite, on the 11th floor, an employee performing construction operations was exposed to falls of approximately one hundred ten (110) feet created by the top rails on the guard rails being at the wrong height and incomplete to protect employees from fall hazards.

29 CFR 1926.502(b)(2)(i): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used between posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide.  a.  On or about April 20, 2017, at the above addressed jobsite, on the 11th floor, an employee performing construction operations was exposed to falls of approximately one hundred ten (110) feet created by the mid rails on the guard rails being at the wrong height and incomplete to protect employees from fall hazards.

On or about April 20, 2017, at the above addressed jobsite, the stairway to the second floor and from floors ten through twelve was not provided with handrails, thereby exposing employees to fall hazards.

On or about April, 20 2017, at the above addressed jobsite, the stairway landing to the 10th floor from the 11th floor, was not provided with guardrail systems, thereby exposing employees to fall hazards.
