29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about April 26, 2017, during the tear off and installation of roofing material on a commercial building, located at 10086 Ellsworth Road, Streetsboro, Ohio, the employer did not ensure employees were protected from a fall hazard of approximately 14 to 16 feet (roof to ground), while working from a roof with a 5/12 pitch (slope), by utilizing a guardrail system, safety net system, and/or other fall protection system or device.
