29 CFR  1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about May 20, 2025, during a residential roofing job at a job site located at 105 S. Mantua Street, Kent, OH 44240, the employer failed to perform inspections of the job site to ensure potential hazardous conditions were identified so that corrective action, such as but not limited to, the implementation and use of personal protective equipment and protection from fall hazards.

29 CFR  1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about May 20, 2025, during a residential roofing job on a sun roof addition, at a job site located at 105 S. Mantua Street, Kent, OH 44240, the employer failed to train employees in the recognition of potential fall hazards and effective means of prevention and protection from such.  Employees are exposed to a fall from height hazard.

Mantua Street, Kent, OH 44240, the employer failed to ensure employees were protected from a fall from a height of approximately 20 feet by the use of  a guardrail system, safety net system, or personal fall arrest system.
