29 CFR 1910.1200(h)(3)(ii): Employees were not trained on the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area:    On or about 3 May 2017, and at times prior thereto, SOT operators were not effectively trained by the employer regarding the physical and health effects of chemicals they are exposed to during cleaning validation, exposing employees to inhalation hazards.
