29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.  On or about 25 April 2020, and at times prior thereto, maintenance employees were cleaning gripper chains on the Global In-Line Foil Machines (GIFM) without applying an energy isolating lockout device, exposing employees to the hazard of being caught-in the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about 25 April 2020, and at times prior thereto, maintenance employees were cleaning gripper chains on the Global In-Line Foil Machines (GIFM) with the machines running and bypassing the safety interlocks, exposing employees to the hazard of being caught-in the exposed moving parts of the machine.
