29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by a fall protection system.  1) Jobsite: An employee engaged in roofing activities approximately 45-50 feet from the ground was exposed to fall hazards without the use of a fall protection system.   This condition was observed 10 MAY 2017.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  1) Jobsite: The employer failed to provide adequate training for employees to recognize and mitigate fall hazards on the jobsite. Employees working on the roof of the building located at 695 N Broad Street Philadelphia PA 19123 did not have fall protection training thus exposing them to fall hazards while working approximately 45-50 feet from the ground.  Condition noted on/about 10 MAY 2017.
