(a) Lou Ritenouer Decorators Inc. dba WR Restoration on or about May 15, 2017, at the address of 141 E. Main St. Ashland, OH: The employer did not ensure an employee tasked with moving platforms to a lower level on a tubular welding frame scaffold was prohibited from utilizing cross braces as a means of access or egress. The employee was exposed to an approximate 20 ft. fall when using the cross braces to access a lower level.

The endpoints at each upright were more than 48 inches (1.3 m) apart.    a) Lou Ritenouer Decorators Inc. dba WR Restoration on or about May 15, 2017, at the address of 141 E. Main St. Ashland, OH: The employer did not ensure employees were adequately protected from falls while working from atop a masonry walkthrough scaffold system. In addition to the rear cross bracing substituting for the top rail, the employer had not installed a mid rail prior to permitting employees to operate atop the 4th frame platform. Employees performing masonry work task in this condition were exposed to a 26 fall to the ground below.
