29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l):    At the location of 110902 W. Riley St. in Overland Park KS, an employee engaged in forklift operation activities, with the use of an All-Terrain forklift (#TL642C) and employees near forklifts on site were exposed to struck-by hazards, in that training was not ensured prior to directing the employee to operate the forklift.
