29 CFR 1910.1030(h)(2)(i)(C): Employer did not ensure that employee training records for occupational exposure to blood or other potentially infectious materials included the names and qualifications of persons conducting the training:  (a) On or about May 29 2017, at Jamaica Hospital Medical Center located at 89-00 Van Wyck Expressway, Jamaica, NY 11418    The employer did not include the names and qualifications of persons providing training information on White Glove Placement, Inc. Orientation Checklist Core Mandatory I: Infection Control / Blood Borne Pathogens, on all employee training records for employees with reasonably anticipated exposure to bloodborne pathogens and other potentially infectious material.   (b) On or about May 29 2017, at Jamaica Hospital Medical Center located at 89-00 Van Wyck Expressway, Jamaica, NY 11418    The employer did not include the names and qualifications of persons providing training information on White Glove Placement, Inc. Core Mandatories I & II Annual Inservice Education Record on all employee training records for employees with reasonably anticipated exposure to blood borne pathogens and other potentially infectious material.      WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
