29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) New residential construction located on Lot #21, Siding cut bench: On or about May 19, 2017, and at times prior, the employer exposed employees to amputation hazards while using circular saws in that the lower guard did not retract immediately, due to wood shims placed between the upper and lower guard to hold it open when the operator cut the Hardie-Plank Siding.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) New residential construction located on Lot #21, North gable side (Pump Jack Scaffold): On or about May 19, 2017, and at times prior, the employer exposed employees to fall hazards of 21-feet, 3-inches to the lower level, in that, a fall protection system was not provided to employees using a pump jack scaffold system to install Hardie-Plank Siding.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) New residential construction on Lot #21, Roof, On or about May 19, 2017, and at times prior, the employer exposed employees to fall hazards of over 25-feet while working on a 5/12 pitched roof, in that an employee wearing a harness was not connected to the life-line provided while using the roof as a walking/working surface to install Hardie-Plank Siding.
