29 CFR 1910.147(a)(2)(ii)(B): An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. On or about 06/02/2017, at 4225 Highway 90 E, Broussard, LA, an employee was operating a Heckert Tung milling machine with a rotating impression blade while using a tool (mallet) to augment a piece of metal dye and was pulled to the point of operation.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: On or about 06/02/2017, at 4225 Highway 90 E, Broussard, LA, an employee was operating a Tung Mill machine that required adjustments during the production process which required the employee to place a tool near the machine's rotating blade.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Four instances of this violation occurred on or about 06/02/2017 in the shop area, at 4225 Highway 90 East, Broussard, LA: a. An employee was operating a Heckert Tung milling machine without a guard to protect the operator from a rotating impression blade to form a metal die. b. A Tong Mill Mud Groover was not guarded to protect the operator from a rotating impression blade to form a metal dye. c. An HEM, Inc. Saw was not guarded to protect the operator from the saw blade while cutting material. d. A DoAll band saw was not guarded to protect the operator from vertical blade while cutting material.
