29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about June 7, 2017, three employees were tearing off and replacing an asphalt shingle roof and they were not protected from falling to the ground. The fall hazard to the employees was at least 18 feet to the ground. The pitch of the main roof was 6 in 12 and the dormer roofs had a pitch of 8 in 12.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill. (a) Jobsite: On or about June 7, 2017, the three employees who were not protected from falling from the roof need to be retained so that they retain the knowledge and skill required to know when and how to properly minimize exposures to falls while working on a roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Jobsite: On or about June 7, 2017, employees were using a 28 foot fiberglass extension ladder to access the roof and the ladder only extended about 1 1/2 feet above the landing. The fall hazard to the employees was about 18 feet to the ground.
