29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas.  On 9 June 2017, three mason workers were removing form boards from the foundation and placing them on the ground with the protruding nails sticking up in the boards, exposing the workers to an impalement hazard to the foot or both feet.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement.  On 9 June 2017, three mason workers were removing formwork from around the foundation of a residential home under construction in which pieces of metal were sticking up at heights around 2.5 to 3 inches and uncapped, exposing the workers to an impalement hazard of tripping and falling onto the metal pieces.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On 9 June 2017, three mason workers were removing form boards by utilizing hammers without wearing eye protection, exposing the workers to the hazard of being struck to the face and eye(s) by flying chips of concrete, particles and debris.
