29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Exigo Metal Works/Hillside, NJ-Throughout facility: The employer did not obtain the truck manufacturer's written approval for use of a front end attachment on the forklift. Employees were using forklifts such as but not limited to Mitsubishi (Model FGC15) with 72 inches long forks attached on the top of the existing forks. Condition noted on or about June 15, 2017.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Exigo Metal Works/Hillside, NJ-Throughout facility: The employer did not mark the truck which was equipped with a front end attachment other than factory installed. Employees were using forklifts such as but not limited to Mitsubishi (Model FGC15) with 72 inches long forks attached on the top of the existing forks. Condition noted on or about June 15, 2017.
